Fish are a symbol of immortality and rebirth, so what I see in this image is a young man seeking the answers of eternal life by attempting to commune with a fish. It suggests seeking the knowledge of health and longevity, but implies a sense of naivete in the bud motif on his tunic.

I think it came up in a more direct sense of naivete about health; or in my case, a cavalier attitude about health.
